Financial Payment Plan - Care Credit

WHAT IS IT?

It is a financial payment plan that one is eligible for with any treatment exceeding $300.00.  It is a plan offered to our patients by a separate company we are set up with;  information is collected by us and given to the company by fax in order to assess whether a patient is covered.  If covered, a monthly payment plan is established.  The first 6 months is interest free, if you require a longer time period as is sometimes the case the interest is as follows.  3% for the first year; 6.5% for 18 months and 12.96% for 24 months or longer which is still less than a regular credit card.  Pamphlets with more information may be found in our office with applications readily on hand.

What do I have to do?

Fill out an application.  (See next page for sample) It is faxed and we will receive a response the same day as to your approval.  In order to apply a patient must have 2 pieces of ID photocopied (1 with photo I.D. such as a Driver’s Licence, Health card, Passport, age of majority card etc…).

How does it work?

It is very easy to apply and establish your Care Credit Account.  Your number of payments and payment amount are established in our office.  We tailor this to your recommendations.  Monthly payments begin 30 days after your treatment is completed.  Payments are pre-authorized by you..
